Inspiration

The Ministry of Health (MOH) reported that one-person households aged 65 and above more than doubled from 42,100 in 2014 to 87,200 in 2024. Despite this, MOH data from October 2024 showed that only one-third of Active Ageing Centres (AACs) reached 30% of the seniors in their areas. We realized that while current indicators prove there are massive outreach gaps, they do not tell coordinators where to look for isolated seniors whose needs remain unseen by standard centre attendance.

What it does

SilverWatch Reach is a data-driven prioritization platform that helps Active Ageing Centre (AAC) coordinators decide where to direct their outreach efforts. It combines an explainable area risk model with a constrained outreach allocator and a follow-up tracking record. Social workers and volunteer coordinators can view the top 10 high-risk planning areas, inspect the underlying evidence, allocate their available team-hours, and track accepted support referrals. It operates entirely on a public-data core without requiring private senior records, allowing staff to retain final operational judgement.|
