Hospitals are currently being penalized 1% of Medicare billing if the fall in the lower tier for 30 day readmission rates, this penalty will increase to 3% by 2016. They lack the tools to follow people after they are discharged from the hospital.In addition, physicians lack to infrastructure to benefit from new billing schemes that Medicare has put in place to help lower readmission rates, such as the Transitional Care Codes.

We knew for a product to have a good fit to help coordinate the care during transitional care that it would have to benefit the patient, the doctor and the hospital.

VideoMD Transitional Care does just that! It benefits the patients with diagnosis specific education, and engagement. Importantly, the program tracks diagnosis specific data points that hospitals and doctors can act upon to improve, and lower 30 day readmission rates. This helps them avoid the Medicare penalty, and improve patient satisfaction. We also help the physicians provide the transitional care, and bill for the new, more lucrative, Transitional Care Codes.

Our revenue comes from hospital and physician fees. Because we are helping hospitals prevent costly readmissions, they will pay a small recurring fee for access to the patient generated data for their high risk patients. For the doctors, since we are helping them increase revenue by billing for the Transitional Care Codes, we will take a small transactional fee.
