The project was born from the ambition to adapt the new frontiers of technology to the European healthcare reality and to promote a solution that reduces the distance among people even in this time of social distancing.

Due to a lack of data about how the clinical program is going on, many patients introduced to a rehabilitation or enablement program don't reach the goal to improve their cognitive/motor skills or they never express their potentiality to the widest possible extent and it dramatically affects their ability to be independent individuals.

The wide use of technologies in the European people's everyday life, such as smartphones, strengthens the feasibility of the proposal, encouraging the adoption of the proposed strategy.

The key achievement of this project is to increase the effectiveness of the rehabilitation/enablement program proposed to a patient after the clinical statement. For complex programs, the statement and the execution of the program are performed by different experts. Here, a lack of communication about the response of the patient to the program is the main reason for the unsuccess of the program itself.

The goal can be reached and it can become a reality by focussing on the effectiveness and readiness of the follow up of the experts (who manage the definition and validation of the rehabilitation/enablement program) with the patients. It can be done if the required time to validate the program is shortened, and it can be reached by sharing a digital tool to monitor the patient experience also remotely which and so providing an increased amount of quantitative data within a system enabled to provide near real-time insights about the patient experience
