Inspiration
Cardiovascular disease affects millions of lives and is the number 1 cause of death in the United States. CVD is increasing very fast. By 2035, 45% of the total American population (about 131 million people), will have at least one health problem related to heart disease and the costs will triple to 1.1 trillion dollars (American Heart Association – AHA).
CVD disrupts people’s lives. Challenges with this are the daily lifestyle and finding a good daily balance between movement, relaxation and food.
A study published by the Centers for Disease Control and Prevention, found that only 34% of patients with heart failure used rehabilitation.
These are missed opportunities to improve patient survival, quality of life, functional status and their cardiovascular risk profile. Studies have shown cardiac rehab programs result in a 26% decline in mortality and can decrease hospital admissions by 31% compared to cardiac patients who don't enroll in a rehab program.
Low participation in cardiac rehab is a constant challenge for many healthcare providers. A huge barrier for many patients is time and motivation. Cardiac rehab usually happens three times a week in two-hour sessions for an average of 36 sessions. People have to go to the hospital, which takes a lot of time and energy.
We are here to make a difference. Many live stream platforms made it for gamers possible to stream, who has high latency and high bandwidth. With T-5Reality we will bring cardiac rehabilitation to everyone’s home, so everyone has access to it.
What it does
Medical professionals like, the cardiologist, physiotherapists and dietitian, can stream their instructions. With rise of 5G and AI on the edge, they can do this with their mobile phone everywhere and anytime. The instructions will be sent to T-5Reality.
The patient can use their Smartphone / Smart TV, or to make it more realistic their XR Glass, where they can interact live with their medical professional.
To give real-time feedback, the patient can connect their smart wearable to the T-5Reality app. This makes it possible that for example the physiotherapists can give real-time feedback.
In addition to streaming, there are games that the medical professional can use during the rehabilitation session. This makes it possible that the patients can do a real-time game, where the medical professional can see directly their biometric data.
All streams and games are stored in the Azure cloud. This makes it possible that the patient can do the exercise again in their own time.

How we built it
Green screen to make instructions video's. Integration with the XR Glass and a Samsung wearable for collecting biometric data.
Challenges we ran into
Connection with the XR Glass. By far the biggest challenge was the time.
Accomplishments that we're proud of
We have build a solution that can help millions of people and gives acces to cardiac rehabilitation to everyone’s home.
What we learned
With 5G we are able to tackle use cases that we otherwise could not. It's eye-opening to see the possibilities and opening up of completely new use cases, thanks to the unparalleled speed of 5G connections.
What's next for T-5Reality: Live rehabilitation to everyone’s home
Do a pilot with cardiologist. COVID-19 affects people heart, where rehabilitation is an important factor. This is the right time to help people.
