Inspiration
Every year, millions of vulnerable patients sign surgical consents, emergency admission forms, and financial waivers without fully understanding what they are signing. Buried inside dense legalese and clinical jargon are predatory covenants: mandatory binding arbitration clauses forfeiting constitutional rights, waivers of jury trials, and unexpected out-of-network balance billing authorizations that bypass federal protections.
For patients under acute clinical stress—and especially non-native English speakers or family caregivers—navigating this paperwork alone is intimidating. We built SubText Health to serve as an empathetic, privacy-first digital patient advocate standing between the healthcare consumer and complex paperwork.
What it does
SubText Health decodes healthcare paperwork, medical bills, and discharge prescriptions in seconds:
- 16-Point Legal & Billing Audit: Identifies violations under the federal No Surprises Act (42 U.S.C. § 300gg-111), unconscionable pre-dispute arbitration clauses, and facility fee upcoding.
- "What Should I Say to the Hospital?": Generates assertive, polite talking scripts patients can read verbatim to hospital administration or financial counselors.
- Multilingual Support: Real-time plain-language translations across English, Sinhala (සිංහල), Tamil (தமிழ்), and Spanish (Español).
- Dedicated Caregiver Action Dashboard: Flips the view to prioritize time-slotted daily medication schedules, critical emergency room red-flag triggers, and verification checklists.
- Interactive Clause Highlighting: Clicking any hazard highlights the exact verbatim excerpt in the original document reference.
- Free Formal Dispute Letter Generation: Instantly produces an editable, statutory-grounded dispute letter demanding itemized UB-04 / CMS-1450 CPT codes and placing accounts on administrative hold.
How we built it
- Frontend: Built with React, Vite, Tailwind CSS, and Lucide Icons, featuring responsive view-toggling between Patient Companion and Caregiver Dashboard.
- Backend: Node.js and Express REST API deployed on Render, processing document payloads, PDF extraction (
pdf-parse), and image uploads. - AI & Multimodal Intelligence: Powered by Groq's high-speed inference engine utilizing Llama models for instant document parsing, statutory analysis, and translation, coupled with
llama-3.2-11b-vision-previewfor scanned document OCR. - Fault-Tolerant Architecture: Implemented a dynamic model resolver and deterministic advocacy safety net to ensure uninterrupted audit generation and zero downtime.
Challenges we ran into
- Ensuring 100% Deterministic Schema Outputs: Balancing complex JSON schemas with deeply nuanced statutory and medical advice across four languages required strict prompt engineering and system-level validation.
- Model Lifecycle & Zero Downtime: Managing dynamic model migrations without breaking frontend state dependencies led us to build a self-healing model resolution layer.
- Client-Side UX for High-Stress Contexts: Designing an interface that presents high-risk warnings clearly without causing panic required careful UX iteration between clinical accuracy and plain-language simplicity.
Accomplishments that we're proud of
- Delivering a comprehensive, multi-perspective committee audit (MD clinical, RN caregiver, JD legal) in under two seconds.
- Building a completely free, editable dispute letter generator that empowers patients to contest arbitrary medical debt.
- Seamless end-to-end multi-language support that translates advice, checklists, and scripts into native scripts while preserving legal citations.
What we learned
- How federal healthcare statutes like the No Surprises Act interact with standard contract law and arbitration agreements.
- Designing privacy-first web apps where sensitive health documents are handled safely in ephemeral session memory.
- Creating effective multi-agent prompt chains that maintain coherent conversational tone across diverse cultural and linguistic backgrounds.
What's next for SubText Health
- Direct hospital chargemaster price comparison and automated CPT code lookup.
- Native mobile camera scanner app with live document edge detection and real-time audio voiceover for visually impaired patients.
- Partnerships with patient advocacy non-profits and legal aid clinics to expand consumer debt defense.
