STORY BEHIND THE CODE: After undergoing a surgery, I found it difficult to manage my medication that I was supposed to take for a few weeks. Having worked in healthcare for over a decade, that got me thinking on how chronic patients manage their meds. My research uncovered clinical, socio-economic, and communication issues faced by the patient and care team. I realized, due to such issues, these patients frequently end up in ED.

I reached out to Alameda County Health System (ACHS) as they were actively trying to address frequent ED utilization problem by pairing patients with health coaches. ACHS clearly saw potential of utilizing mobile and web technology to assist patient and health coaches and agreed to become SimplifiMed's pilot site.

TARGET USER: Anyone with chronic condition can use SimplifiMed app for medication management. This includes medication reminder, reauthorization, refill, and copay tracking. The app also enables patient to keep a list of providers, address, and their phone number. From this list, patient can call the physician office, or plan route to the office.

A hospital using social workers (such as care coaches at ACHS) provides SimplifiMed desktop application and app to manage super utilizers such as Mary to follow treatment plan. The app allows social worker to securely communicate with the patient while the desktop enable her to track patient's adherence to medication, create and manage a care plan, enroll patient in free/subsidized medication plan etc.
