Working as a physical therapist for 25 years with marginalized communities and with the geriatric population along with our team with healthcare industry experiences, we have witnessed the challenges a patient faces with lack of access to community resources after being discharged from the hospital or emergency room. Health inequalities which cause certain patient population lesser access to the healthcare systems, often results in costly readmission. It also decreases a patient’s ability to transition successfully into the community and to manage their health and well-being.
