Inspiration
Industrial safety decisions often require engineers and safety professionals to work across a large amount of fragmented information: Standard Operating Procedures (SOPs), P&IDs, Management of Change (MOC) history, Hazard and Operability Study (HAZOP) records, work-permit requirements, applicable regulations, equipment specifications, and previous incidents.
The problem is not simply finding a document. The difficult part is connecting the right evidence from multiple sources and turning it into a decision that a safety professional can actually trust.
We wanted to build an AI system that does more than answer questions. SafeOps AI was designed to investigate a safety situation, find supporting evidence, identify relevant requirements and risks, and produce an explainable recommendation that can be reviewed by a human safety professional.
What it does
SafeOps AI is an evidence-backed industrial safety agent.
A user can provide an industrial safety scenario, incident, maintenance activity, or operational question. The agent analyzes the situation against relevant technical and safety information.
The workflow is:
Safety scenario → Evidence retrieval → Technical document analysis → Requirement checking → Risk identification → Evidence-backed recommendation → Human review
SafeOps AI can work across information such as:
- Standard Operating Procedures (SOPs)
- Piping and Instrumentation Diagrams (P&IDs)
- Management of Change (MOC) history
- Hazard and Operability Study (HAZOP) information
- Work-permit requirements
- Applicable safety and regulatory requirements
- Equipment and material specifications
- Previous similar incidents and events
Instead of returning an unsupported AI-generated answer, SafeOps AI is designed to show why a recommendation was made and which evidence supports it.
The final output can help a safety professional understand:
- What the situation is
- What requirements are relevant
- What risks or inconsistencies were identified
- What evidence supports the findings
- What additional information may be required
- What action or recommendation should be considered
The goal is not to replace the safety professional. The goal is to reduce the time spent searching, comparing, and interpreting fragmented information so that experts can focus on the final decision.
How we built it
We built SafeOps AI around an agentic workflow rather than a simple conversational interface.
The system separates the process into multiple stages:
- Understand the request — identify the operational situation, equipment, activity, and safety question.
- Retrieve relevant evidence — locate information from the available technical and safety knowledge sources.
- Analyze the evidence — compare information across documents and identify relevant requirements, conditions, and potential conflicts.
- Reason over the evidence — determine how the findings relate to the specific safety scenario.
- Generate an advisory — produce a structured recommendation with supporting evidence.
- Human-in-the-loop review — keep the final safety decision with a qualified human rather than allowing the AI to make an autonomous safety-critical decision.
- Maintain traceability — preserve the reasoning and supporting evidence so the recommendation can be reviewed.
The architecture was designed around a simple principle:
AI can accelerate investigation, but safety decisions must remain evidence-backed and reviewable.
Challenges we ran into
The biggest challenge was preventing the system from behaving like a generic AI chatbot.
Industrial safety information is highly contextual. A recommendation can depend on the exact equipment, operating condition, procedure, previous modification, permit requirements, or applicable regulation.
Another challenge was handling conflicting or incomplete information. Instead of assuming that missing information is correct, SafeOps AI is designed to identify uncertainty and surface areas that require human verification.
We also had to balance automation with safety. A system that confidently produces an answer without showing its evidence can create a false sense of certainty. That is why we focused on evidence, traceability, and human review as core parts of the product rather than optional features.
Accomplishments that we're proud of
We are proud that SafeOps AI goes beyond the typical “ask an AI a question” experience.
The project demonstrates how an AI agent can transform a complex industrial investigation into a structured workflow:
Question → Evidence → Cross-check → Risk → Recommendation → Human verification
We also focused on making the system useful for a real operational environment rather than building a purely theoretical AI demonstration.
Our strongest design decision was making evidence-backed reasoning a first-class part of the experience. The system is not intended to simply sound intelligent — it is intended to make its recommendations easier to investigate and verify.
What we learned
We learned that building an AI agent for a high-consequence domain is very different from building a general-purpose chatbot.
The quality of the final answer depends heavily on:
- The quality of the underlying information
- Retrieval of the right evidence
- Context preservation
- Clear separation between evidence and AI reasoning
- Handling uncertainty
- Human verification
- Traceability of recommendations
We also learned that the most valuable role for AI is often not replacing an expert, but compressing hours of information discovery and analysis into a workflow that lets the expert make a better-informed decision faster.
What's next for SafeOps AI
The next step is to expand SafeOps AI from an evidence-backed advisory system into a broader industrial safety intelligence platform.
Future versions could include:
- Deeper integration with enterprise safety and maintenance systems
- Automated document and knowledge-base ingestion
- More advanced regulatory and standards mapping
- Historical incident pattern analysis
- Risk trend detection
- Automated safety review checklists
- Stronger audit trails
- Role-based workflows for engineers, safety officers, and auditors
- Human approval workflows for safety-critical recommendations
Our long-term vision is simple:
Give industrial safety professionals an AI copilot that can investigate thousands of pages of technical information quickly — while keeping evidence, accountability, and human judgment at the center of every important decision.
