Inspiration
Proactive holistic health Chinese health model - pay GP's to KEEP you healthy (proactive) as opposed to pay a GP to treat you when you're sick (reactive). The role of the doctor is in overall health. Doctor is paid on a monthly retainer as long as the patient remains healthy. Preventative health care - decrease chronic preventable disease and therefore increased quality of life/longevity during ageing and therefore greater chance of ageing at home. Change focus from cure to prevention to maintaining general wellbeing. Plan is to look at the health of the person in the body and happiness in the mind. Latest advances in tech but moving away from the core question - what is the mission of the health provider/system in the field of medicine? Realigning focus. Create intergenerational communities - elderly live alongside the general public include youth.
What it does
Flip the current reactive health care model to a proactive preventative model. Treat healthy people on a monthly membership to ensure they remain well, live a life full of good health and purpose and remain out of aged care facilities towards the later years. Person signs up to our membership, pays fee, and completes a healthcare assessment. This assessment is sent to case management workers (supporting and employing burnt out and retired health care workers - social enterprise aspect) who act as a coach and do a 1:1 welcome and further health assessment touchpoint, IF THEY need further assessment then patient goes to care team and has an assessment with an RN & GP resulting in a personalized care plan which is monitored with check-ins by the case management worker.
How we built it
Comparing current Australian models against international alternatives Collating the concept of multidisciplinary teams and centralized shared care to a preventative health setting in a more efficient and effective way that is more economically viable
Challenges we ran into
Health care subscription model - unaffordable to low socioeconomic, removing health professionals from the current system which is already struggling with lack of care providers, Missing the mark with the immediate ageing population - eg. shooting for the stars but missing the moon. Potential exclusion of those currently unwell or suffering chronic disease and only serving the existing healthy population. Potential exclusion of those unable to afford the service/low socioeconomic demographic who would most benefit from the proactive health care model. Therefore only serving wealthy people who are statistically less likely to suffer from preventable health conditions. Copying/competing with current community services. Medico-legal implications with regard to what could be "preventable"
