MyNote

MyNote is a SMART app designed to function inside a SMART PHR or EMR.  MyNote provides an intuitive, interactive timeline of patient history with disease-specific schemes, allows patients to add temporal notes to any problem or medication, and ultimately facilitates physician assessment and patient self-management of patient health patterns and drug responses.
