MediMind
AI-Powered Clinical Patient Intake & Intelligence Agent
MediMind is an AI-powered clinical intake assistant that transforms unstructured patient records into structured, doctor-ready clinical summaries while enabling grounded question answering over patient documents.
Unlike a generic medical chatbot, MediMind is designed to operate only on the uploaded clinical records, ensuring that every response is traceable, grounded, and auditable.
Problem Statement
Doctors often spend 10–20 minutes manually reviewing patient records before making clinical decisions.
These records may include:
- Discharge summaries
- Laboratory reports
- Clinical notes
- Imaging reports
- Medication history
- Follow-up records
This leads to:
- Increased cognitive load
- Delayed clinical decisions
- Information overload
- Missed critical observations
Our Solution
MediMind automatically:
- Reads patient documents
- Generates a structured clinical intake summary
- Extracts medications, symptoms, procedures, and medical history
- Highlights clinically important observations
- Answers doctor questions using only the uploaded records
- Supports analysis across multiple patient documents
- Designed to integrate with live document updates through Pathway

Formal Representation
Let
[ D={d_1,d_2,\dots,d_n} ]
represent the uploaded patient documents.
The structured clinical summary is generated as
[ S=f(D) ]
where
- (D) = patient records
- (f) = constrained clinical intake generation
Doctor queries are answered using
[ A=g(S,q) ]
where
- (S) = structured intake summary
- (q) = clinician query
- (A) = grounded response
Our objective is
[ A \subseteq D ]
meaning every generated answer must be supported by information contained within the uploaded records.
