Inspiration

Being healthcare professionals, we frequently see patients struggling with polypharmacy — managing multiple medications prescribed by different providers. Long and complex medication names, different dosing rules, and poor coordination between care teams often lead to medication errors and poor adherence. We wanted to build something simple, personal, and reliable that helps patients integrate medication into their daily lives.

What it does

MediMate is a mockup prototype that demonstrates how a mobile app could:

Consolidates all medications into a single, simplified list.
Provides a personalized schedule with reminders and pill images.
Tracks pill counts automatically so patients know when supplies are running low.
Offers quick access to medication information (indications, dosing, side effects).
Integrates with pharmacies/healthcare systems via NFC tap-to-sync, keeping providers updated and reducing errors.

How we built it

Designed and developed high-fidelity mockups to visualize the user journey using Sketch.
Focused on UI/UX design to ensure it’s simple for elderly patients and practical for providers.
Created workflow screens (login via Singpass, medication list, schedule, pill counter, refill process).
Incorporated design elements based on real patient scenarios to ensure relevance.
