Inspiration
Our inspiration for the tool was the emerging implementation of the patient-centered medical home (PCMH) model for practice redesign. This model makes care more patient-centered, leading to improved outcomes and efficiency. However, practices struggle to determine what it will cost to transform their practice.
How it works
The tool is an Excel spreadsheet. Practices answer a number of questions about their practice, how they plan to transform, and financial resources available to them. The tool estimates the cost of transformation, and how valuable their practice becomes when it is a PCMH.
Challenges I ran into
Practices struggled to enter the necessary financial and clinical data. The tool was generally too complex and time intensive for them to use.
Accomplishments that I'm proud of
We collected data about the cost and value of PCMH for small practices that is unmatched by any existing efforts, and created an excellent alpha version of our tool.
What I learned
Future versions of the tool need to be much more user friendly. They also need to include the use of auto-populated data to pull in information directly from a practice electronic medical record and administrative data in order to make the tool easier to use.
What's next for JUMP COST Tool
We are considering two new versions of the tool. One would be a simplified, five to six question app that would give practices a high level estimate of the cost and value of PCMH implementation. Another would be an online tool, similar to an online tax preparation software, that would facilitate data entry for the practices and allow them to play around with the numbers to see how they can make practice change more valuable for their practice.
