Your best daily five minutes on your heart health journey. We put a patient's doctor on their kitchen table, and pay their doctor to do it. HEARTPartner helps patients complete their daily risk assessment while building skills in diet, physical activity, taking their meds and writing stuff down. We're building on 24 months of experience delivering a coordinated care program for diabetes in partnership with the American Diabetes Association, changing patient behavior "beyond the pill".
We've been working hard the past two years in partnership with the American Diabetes Association to help seniors build core skills in diet, physical activity, taking their meds and writing stuff down. Our incredibly caring nursing team paired with our digital experience impacted A1Cs, weight, activity levels and longevity.
About a year ago we presented some of our patients with some potential innovations we were considering:
- An Apple Watch implementation -- too small!
- An enhancement to the mobile experience that "read" the numbers off a glucometer face -- failed when we handed it to twenty seniors
- A voice-based coordinated care service accessed through Alexa -- it rated a 6 out of 10, mostly with "I don't get it."
So we sent ten seniors home with a Alexa-voice prototype and... two weeks later the feedback was "15 out of 10, how can I get this?".
As we prepared to launch the Voice experience for diabetes we also were completing a product review with our patients, and (not surprisingly) heart disease is the #1 co-morbidity with diabetes. We began to build out a program that expanded to include the central measures of CHF -- weight, breathing, edema -- and expanded our hardware connectivity to include home pressure cuffs and scales.
We are now preparing to launch HEARTPartner, a voice based coordinated care service connected to your doctor's EMR. We deliver personalized support and physicians call bill Medicare $40-$60/month for every patient that utilizes our service.
What it does
With the simple phrase "Alexa, check in with heart partner" we help our patients complete their daily assessment and build core skills in diet, physical activity, taking their meds and writing stuff down. We're connected to all their devices and to their physician's EMR.
What's next for HEARTPartner
Our next steps will be extended EMR integrations, expanded meal planning integrations and exercise programs.
