Amidst growing social isolation risks among the elderly as Singapore moves towards a super-aged society, community care organisations navigating challenges such as limited social workers, volunteers and outreach capacity need to regularly identify and reprioritise planning areas based on current and future potential risk, existing support coverage and available resources. Heartland Kakis helps social workers identify which planning areas require the most urgent attention, understand why they are prioritised, and decide what type of support may be appropriate.
