AI-powered continuity of care platform connecting hospitals, patients, and pharmacies


▸ Inspiration

Post-hospital care is often fragmented—patients forget medications, miss follow-ups, and lack continuous monitoring. Studies show that nearly (43%) of patients miss medications within 30 days of discharge. This gap inspired us to build a system that ensures continuity of care beyond hospital walls.


▸ What it does

HealthSync creates a connected ecosystem between patients, doctors, and pharmacies.

  • Digitizes prescriptions and lab reports
  • Sends smart medication reminders
  • Flags abnormal lab values using AI
  • Monitors vitals in real time
  • Predicts emergencies before they occur

This ensures proactive, data-driven recovery at home.

  • Frontend: React + Vite + Tailwind CSS
  • Backend: Node.js (Firebase planned)
  • AI Stack: TensorFlow, spaCy, OCR pipelines
  • Integrations: Wearables and health APIs

AI models process medical data and generate actionable insights, while real-time systems handle alerts and monitoring.
