Executive Summary

Covid19 has crippled people’s lives and businesses in an unprecedented way. World is waking up to the new reality of isolation, self-quarantine and social distancing. Some of the most robust healthcare systems around the globe have been overwhelmed by the rapid onset of this pandemic.

Triage is a key strategy deployed to segregate the suspected COVID19 patients from other patients to thwart further outbreaks. To make the triage of massive influx of patients manageable, healthcare systems advise patients to call-in a healthcare hotline, through which healthcare professionals conduct triage remotely, before visiting healthcare facilities. However, these hotlines usually get inundated, limiting the healthcare authority’s ability to assess the cases in detail.

HealthVision by MillenniumIT ESP provides a web-enabled, mobile responsive, intuitive platform for healthcare authorities to get connected with suspected patients remotely, record their symptoms through a guided questionnaire, visual capture of respiratory symptoms and their symptom history to ascertain the likelihood of a COVID19 infection. Public can securely access HealthVision platform through a URL and use an OTP sent across to their mobile for authentication. Each recorded case will be identified with a unique case number which is provided to the user for future reference.

COVID19 leaves a large segment of the population carrying a high risk exposure either in quarantine or in self-isolation. Health authorities maintain a continuous vigil of these high-risk categories through physical contact by the health inspectors and remote monitoring. HealthVision can be effectively deployed to maintain a continuous vigil of these high-risk groups remotely, assessing their symptoms progressively and help identify the likely COVID19 cases. These quarantined/ isolated users can return to the platform and use the unique case number in possession to update their symptoms progressively.

HealthVision provides the healthcare authorities a detailed information of symptoms and video captures of the cases, risk ratings to prioritise the cases and the ability to customize questionnaires and workflows. In addition, HealthVision automatically allocates cases to healthcare professions allocated in the system. Taking time criticality of the current situation into consideration, MillenniumIT EPS has developed a minimum viable product (MVP) and will continue to add functionality and features to automate further and to provide critical insights and outcomes to healthcare authorities with the use of analytics and artificial intelligence. Below section on HealthVisio Analytics and HealthVision Automation describes the added functionality of HealthVisio Version 1.0 & Version 2.0

HealthVisio Analytics (V1.0)

HealthVision Analytics provide critical insights for healthcare authorities to prioritize their actions based on risk rating of the reported cases. The symptom rating engine will take patient demographics, Incumbent health conditions, recent social social exposure, current symptoms and the symptom history to derive at the overall risk rating of individual cases. Curated information is presented to healthcare authorities through a dashboard. HealthVision Analytics also comprises of a categorization engine to categorize cases based on the overall risk of the case as well as based on similarities of the symptoms. This will greatly improve the efficiency of the healthcare authorise to prioritize and respond to cases.

HealthVision provides functionality for healthcare professionals to respond to cases through text-based instructions and through a brief audio/video recording. These medical instructions can be accessed by the corresponding members of the public as they login to the platform. HealthVision provides a two-way communication between the public and healthcare authorities throughout the lifecycle of the case.

One of the key advantages of HealthVision is the ability it provides to healthcare professionals to respond to groups/ categories of cases with similar risk rating or symptoms, with a single response of medical instructions. This will greatly improve the healthcare systems efficiency and thereby its capacity to manage the biggest challenge posed by COVID19, overwhelming of healthcare system’s capacity.

HealthVision Automation (V2.0)

HealthVision Automation extends the capability of the platform to provide autonomous responses to cases without the intervention of healthcare professionals with the use of AI. This functionality can be deployed for low-risk and medium-risk categories, freeing up healthcare professionals, to focus more on critical cases. HealthVision Automation also provides a workflow engine to automate peripheral functions such as notifications/information/transactions to third party systems.

COVID19 even took some of the global pandemic experts by surprise while general public is still struggling to come to terms with this new reality. Throughout the history, humans have conquered many catastrophic threats and we will respond to COVID19 with a renewed global unity, solidarity and with an unwavering resolve of political, business and social leadership.

Among many tools we possess to fight against COVID19, technology plays a key role. Technology connects and brings people together, while maintaining a physical social distance. Today we bring employees, businesses, families, governments together through technology to minimize the impact of social distancing. Bringing patients and healthcare professionals together with technology improves the efficiency of healthcare response against COVID19, while safeguarding both patients and healthcare professionals. The patients will benefit from quick and timely responses while healthcare professionals will benefit from lowering their risk of exposure.

Even in Sri Lanka is currently in stage of community spread with nearly 300 suspected cases under hospitalization. The government has deployed an effective social distancing strategy together with suspect tracing and isolation mechanism to combat COVID19.

The objective of the government is to contain the community spread and to flatten the curve.

Current Challenges

Among many response mechanisms, the government has rolled out an effective emergency response for suspected COVID19 patients. Public is given many emergency numbers for inquiries, remote assessments and emergency response to patients with advance symptoms. We foresee a few challenges in this approach if the number of suspected / self-quarantined cases increase rapidly.

  • When number of calls increase the medical agent will not be able to attend each and every call, resulting in mishandled cases.
  • There is no proper track record of each person’s inquiry or continuous assessment of the progression or follow up.
  • Getting information through a voice call would not be sufficient to assess the status/critical factor of the suspected case.

Our Solution

MillenniumIT ESPs HealthVision platform envisages to connect the public with the government medical emergency centre via a digital platform. Health Vision can assess current symptoms, symptom history and capture visual responses of primary diagnostics of suspected patients and provide risk scores and other insights for healthcare professionals to triage and prioritize their response. Health Vision can be extended to response automation and case management if the need be. Health Vision solution consists of mainly 2 modules

Medical personal portal

Medical personal portal would enable functionalities for medical agents to structure how the assessment needs to be automatically conducted by a workflow. There will be a default set of question workflow’s built with the analysis done by the MillenniumIT ESP business analysis team on the current situation. The medical users can edit and structure of the assessments according to how they need. These assessment flows can be in the form of text fields, radio buttons, drop down as well as video & voice recordings. When public users starts submitting their profiles the medical agents would get notified and they would be able to view and analyse the situation of the public. Medical agents would be able to replay and compare submissions from the public to decide where potential threats can happen in the future as well as communicate with the public on how to further stay safe and which steps to be taken. The medical users can also add their comments and rate the critical factors of the patients according to the provided data.

Patient assessment portal

The public can simply access the portal via smart phone , tab or pc and sign in with their mobile number. The user would get a onetime password to be entered and once that is done the sign in process is completed. Once signed in an initial set of questions will be prompt to be answered by the user which is introduced as “stage 1” assessment. Once this is completed the user would be redirected to the 2nd & 3rd stage where users would answer via video and audio as well. Once the assessment is done the user can simply log out and the agents will get back to the user on the next steps.

Impact of the solution

  1. Enabling a digital channel for the public to communicate with the government covid19 emergency centre.
  2. Automating first level covid19 assessment and easing out the pressure for the government medical agents by handling the incoming load.
  3. Providing more valuable data points about the public who are under self-quarantined and self-isolation. (currently only calls but no mechanism / platform to gathering data)
  4. Helping the emergency medical team to take quick action on potential future outbreaks
  5. Comparison and recap of public user submitted profile videos.

Rollout plan

Phase 1 Above mentioned MVP

Phase 2 [After 1000 profile submissions] Machine Learning classification model to auto rate the critical factors of the patients according to the manual rating done by the medical agents on the platform in phase 1. This data would be used to train the ML model for future predictions including auto classifying patients and potential future outbreaks
