Health Monitor Notification

Inspiration

My mother is diabetic. She used to visit her primary physician once every 3 months to get her blood work tested. The provider has access to the blood work done at the hospital/clinic. My mom has a home kit which she tests regularly at home. Her primary physician does not have access to her home kit monitor. I see a gap here.

What it does

I designed to build an integration layer that will establish a connection between the home kit monitoring tool and the provider database so that the primary physician can access real-time data of their patient.

What's next for Health Monitor Notification

Build email notifications to primary physicians about their patients when the home kit readings trend upwards or in the danger zone.
