Inspiration
Healthcare still relies heavily on paper-based consent forms and scattered medical records. Patients may sign complex medical documents without fully understanding them, while doctors and hospitals can face difficulties managing, retrieving, and tracking sensitive records. We were inspired to build a secure platform that makes consent more understandable and medical documentation more organized, accessible, and accountable.
What it does
SMART CONSENT & PATIENT MEDICAL DOCUMENTATION PORTAL provides a centralized platform for managing patient medical documents and digital consent.
Key features include:
- Secure patient medical document management
- Digital consent creation and approval
- AI-powered explanation of complex medical terminology
- AI-assisted document summarization
- Consent version tracking and lifecycle management
- Role-based access for patients, doctors, admins, and auditors
- Secure document storage and controlled access
- Complete audit trail of document and consent activities
- Consent expiry and revocation management
- Emergency access with accountability
- Notifications and security alerts
How we built it
We built the application using a modern full-stack architecture.
Frontend: React.js, Vite, Tailwind CSS, React Router, Axios and Recharts
Backend: Node.js, Express.js and REST APIs
Database:MongoDB Atlas
File Storage: Amazon S3 for secure medical document storage
AI:Google Gemini API for consent explanation, medical terminology clarification and document summarization
Security:JWT authentication, password hashing, role-based access control, input validation, rate limiting, private storage and audit logging Additional Technologies:Tesseract OCR, Nodemailer, Git and GitHub
The application follows a modular architecture where medical document metadata is stored in the database while actual files are securely stored in private object storage.
Challenges we ran into
The biggest challenge was designing a system that balances **AI assistance, healthcare privacy, security, and usability.
We had to ensure that:
- Patients can only access their authorized records.
- Sensitive documents are not publicly accessible.
- Consent versions cannot be accidentally overwritten.
- Every important action is traceable through audit logs.
- AI explanations do not introduce medical misinformation.
- AI assists patients in understanding information rather than making medical decisions.
- Different users have different levels of access.
- Emergency access remains controlled and accountable.
Accomplishments that we're proud of
We are proud of transforming a simple document-management concept into a complete healthcare workflow.
Our major accomplishments include:
- Building a secure digital consent lifecycle
- Introducing AI-assisted patient-friendly consent explanations
- Implementing centralized medical document management
- Creating version-controlled consent records
- Implementing role-based access control
- Building a detailed audit trail
- Adding controlled emergency access
- Creating security monitoring and access alerts
- Designing a professional and responsive healthcare interface
Most importantly, the project focuses on patient understanding and control, rather than simply digitizing paperwork.
What we learned
Through this project, we learned that building a healthcare application requires much more than creating CRUD APIs.
We learned about:
- Secure authentication and authorization
- Healthcare-oriented data privacy
- Cloud object storage
- Secure file access
- API design and backend architecture
- Database relationships and data modeling
- AI integration and responsible AI usage
- Document processing and OCR
- Audit logging
- Version control for sensitive records
- Designing applications around real-world user workflows
We also learned that AI should be used carefully in healthcare, where accuracy, transparency, privacy, and human oversight are critical.
What's next for SMART CONSENT & PATIENT MEDICAL DOCUMENTATION PORTAL
Our future roadmap includes:
- Advanced OCR for scanned medical records
- Semantic search across medical documents
- AI-powered document classification
- More sophisticated abnormal-access detection
- Mobile application for patients and doctors
- Multilingual consent explanations for regional languages
- Integration with hospital information systems
- Interoperability using healthcare standards such as FHIR
- Stronger electronic signature integration
- Advanced analytics for hospital administrators
- More granular consent and data-sharing controls
Our long-term vision is to evolve SMART CONSENT into a secure, patient-centric digital healthcare documentation and consent ecosystem that improves transparency, understanding, and trust between patients and healthcare providers.
