🛡️ GuardianAI: Real-Time Clinical Handoff Assistant
AI-powered system that prevents hospital handoff errors and saves lives through real-time clinical intelligence.
🚀 Inspiration
Every year, thousands of patients die from something that should never happen — miscommunication during nurse shift handoffs.
Even with modern EHR systems, critical details like allergies, pending lab results, or outdated code statuses often get lost in transition.
We wanted to build an AI system that acts as a guardian angel during these moments — one that never forgets, never tires, and always watches over patient safety.
🤖 What It Does
GuardianAI is an AI-powered clinical handoff assistant that continuously analyzes patient data during shift changes to prevent critical errors.
Key Features:
- 🔍 Reviews EHRs, lab reports, and clinical notes for missing or outdated info
- 🚨 Flags critical gaps (e.g., unrecorded allergies, pending labs, outdated code status)
- ⚡ Sends instant alerts to day and night nurses during handoff
- 🧠 Continuously learns from feedback to improve accuracy
- 💬 Monitors ongoing updates and escalates urgent alerts automatically
✅ Result: No more forgotten details. No more preventable harm.
100% focus on what matters most — saving lives.
🏗️ How We Built It
- Frontend: React.js for the interactive Bed Board and Live Feed dashboard
- Backend: StackAI for HIPAA-compliant workflow orchestration
- Data Processing: Apify for real-time medical data gathering and knowledge verification
- AI Engine: Airia for multi-model routing and dynamic cost optimization (70% cost savings)
- Visualization: Real-time live feed updates with dynamic learning weight bars for continuous AI training
The system dynamically adjusts “learning weights” like Allergy Missing, Pending Labs, and Code Status Outdated based on previous outcomes and nurse responses.
🧩 Challenges We Ran Into
- Synchronizing real-time AI analysis with live dashboard updates
- Ensuring HIPAA compliance across all data pipelines
- Designing an alert system that’s helpful but not overwhelming for clinicians
- Managing AI workflow retries and error handling under simulated load
🏆 Accomplishments We’re Proud Of
- Built a fully functional real-time hospital dashboard simulation
- Achieved 100% detection of simulated allergy and lab data gaps
- Integrated Apify, StackAI, and Airia seamlessly
- Reduced AI inference cost by 70% through smart routing
- Created an intuitive UI that mimics real-world hospital environments
📚 What We Learned
- How to orchestrate multiple AI agents for healthcare-grade reliability
- The power of feedback-driven AI learning in reducing medical risks
- The importance of clear, minimal UI feedback in high-stress environments
- That technology alone isn’t enough — designing for human trust is essential in healthcare
🔮 What’s Next for GuardianAI
- ✅ Integrate with live EHR systems (Epic, Cerner) via secure APIs
- 🩺 Add more alert types (e.g., medication conflicts, vital anomalies)
- 🏥 Deploy a pilot version in a teaching hospital
- 🗣️ Add voice assistant integration for hands-free nurse interaction
- ⚖️ Begin FDA & HIPAA certification process for real-world deployment
💡 Team Vision
GuardianAI stands for one simple mission —
Where technology meets compassion to save lives.

🏥 Demo Scenario: Mrs. Johnson Case
- 7:00 PM — Nurse handoff begins
- AI detects missing peanut allergy → sends instant alert
- 9:47 PM — Lab shows abnormal cardiac enzymes → “Call doctor NOW” alert
- Surgery rescheduled safely → life saved, zero incident
Result: 100% preventable incident avoided.
GuardianAI proves that smart handoffs save lives.
