Inspiration

I started this project after looking at the 99+ forms on FEMA's website for disaster alone. The sheer quantity of the forms makes it very confusing to fill out especially after being in a disaster yourself. These people need help and support quickly, but this long and arduous process makes it impossible. So I sought after a way to stop this and quicken the process.

What it does

“Fast Forms locates you, identifies your local Disaster Assistance Center (DAC), allows you to begin filling out forms, and tracks your status.”

A DROP-IN SOLUTION THAT WORKS WITH THE EXISTING PAPER/PDF PROCESS USED BY DACS.

  1. Start with your mobile phone, enter you information once.
  2. Arrive at your chosen assistance center to complete the process.
  3. After your forms are submitted, track and get notifications of the status.
  4. The DAC will know your ETA and have the dozens of forms pre-filled out when you arrive.
  5. With your ETA, the DAC will have on demand staffing for the volunteers.

How I built it

I built it from the ground up and used HTML, JavaScript, CSS, and Google-Map API.

What's next for Fast Form | Disaster Relief Form Editor

Next is to expand the website to the government and let them use it in cases of disaster.
