Inspiration

COVID–19 is accelerating the pace at which organizations need to rapidly enable & streamline remote work. Many organizations are not yet equipped to handle the current situation. We plan to do our part to help.

What it does

Health departments are completing the COVID-19 Case Report form manually by downloading this PDF https://www.cdc.gov/coronavirus/2019-ncov/downloads/pui-form.pdf from CDC’s website. The pui-form.pdf is submitted by health departments to their respective States and CDC in order for the CDC to track the outbreak and inform a public health response.

Our team created an electronic version of CDC's COVID-19 Case Report form.

Our eform meets Section 508 and WCAG 2.0 accessibility standards; works on both desktop browser and all mobile devices; enforces field validation; contains business logic that makes the form much easier and faster to fill then the manual form pui-form.pdf; contains an electronic signature. Our eform can output XML data any XSD format required to interoperate with healthcare standards.
