ChroniCare

Inspiration - The lack of resources and followup care available to chronically ill patients in rural communities.

What it does - This app connects patients to a network of local physicians and allows for tracking vital signs. This will allow for rural communities to become self-reliant.

How we built it - Designed the app by using Proto.io. Presented using PowerPoint.

Challenges we ran into - No technical expertise on team and finding a single part of the problem to focus on.

Accomplishments that we're proud of - Created a great prototype and presentation with no technical expertise.

What we learned - How to create prototypes and create self-sustaining solutions. Learned to look at solutions holistically.

What's next for ChroniCare - Partnering with a technical team to create an app in phase two and then initiating a trial run in phase 3.
