Inspiration
Chronic diseases (hyperlipidemia, hypertension, diabetes) are becoming more prevalent and life threatening than ever before. On average, we are consuming 8 times the suggested amout of sugar daily and 95% of us ecceed normal salt intake, which could easily lead to 5kg yearly weight gain, obeisity, even death. While Singapore continuously advocates healthy eating, exercise and regular health check-ups, 39% of residents suffer from hyperlipidemia and 31% from hypertention alone. That is where the idea of Chronic Catchment came from, to provide a healthcare safety net for our elders in singapore, who make up 21% of our population, starting from the simplest yet most common diseases.
But where does this safety net come from?
MOH has been putting in consistent effort in crafting a nurturing healthcare community for every planning area, and some would even say we are quite proud of our comminity healthcare, but is that really the case?Browsing through datasets, I discovered huge hidden gaps that we have been blinded from: hospital admissions : polyclinic admissions > 20 : 1, 10 times more costly private clinics than polyclinics. However, what struck me the most was that these services were always there, especially polyclinic services, but with just 26 functioning polyclinics among our 55 planning areas, they were just too vastly apart such that elders and low income families could not afford the transport to reach them.
That's when Chronic Cathment became Chronic Catchment on Wheels. Not only are we going to close the community healthcare gap, but we are also going to get seniors and struggling families to the healthcare support that they need...
What it does
• 5-year chronic-disease demand forecast by planning area
• Citizen registration flow: simulated SingPass + medical history + dietary profile
• Nearest-capable-polyclinic matching engine, powered by our HPB service-tag data
• Live transport-subsidy eligibility engine, end-to-end
• Genie Space for policymakers to query the forecast in natural language
• Volunteer matching for bedridden elderly
• Chatbot to provide everyday dietary recommendations and educational MOH policy videos to raise social awareness
