CareTrack – Post-Operative Care & Recovery Management Platform
About the Project
💡 Inspiration
Post-operative care does not end when a patient leaves the hospital. After discharge, patients may forget medication schedules, miss follow-up appointments, or fail to communicate early symptoms to healthcare professionals. Healthcare professionals may also have limited visibility into a patient's recovery between hospital visits.
These challenges inspired us to build CareTrack, a centralized digital platform designed to support patients and healthcare professionals throughout the post-operative recovery journey.
Our goal is to make post-operative care more organized, continuous, and easier to monitor after hospital discharge.
🎯 What We Built
CareTrack integrates multiple post-operative care activities into a single platform:
Personalized Discharge Planning – Provides patients with customized wound-care, diet, activity, medication, and follow-up instructions.
Medication Monitoring – Helps patients track medication schedules, dosage, timing, and adherence.
Recovery Tracking – Allows patients to record pain levels, symptoms, recovery status, and progress.
Automated Follow-Up Management – Helps manage appointments, reminders, and follow-up history.
Patient Self-Reporting – Enables patients to submit symptoms and recovery updates remotely.
Early Risk Alerts – Identifies potentially concerning patient-reported information and brings it to the attention of healthcare professionals.
Healthcare Professional Dashboard – Provides a centralized view of patients, recovery information, medications, appointments, and alerts.
The overall objective is to provide continuous post-operative support beyond hospital discharge rather than relying only on periodic hospital visits.
🏗️ How We Built It
We designed the system using a layered architecture consisting of:
Presentation Layer → Application Layer → Data Layer
The presentation layer provides separate interfaces for patients and healthcare professionals. The application layer handles major functions such as discharge planning, follow-up management, medication monitoring, recovery tracking, risk assessment, and notification management. The data layer is designed to maintain patient, surgery, discharge, medication, recovery, follow-up, and alert information.
The main entities in our database design include:
Patient
Healthcare Professional
Surgery
Discharge Plan
Medication
Recovery Record
Follow-Up Appointment
Risk Alert
These entities are connected to maintain a centralized view of the patient's post-operative journey.
📚 What We Learned
Through this project, we learned how to:
Analyze real-world healthcare problems and convert them into software requirements.
Design a healthcare-focused user interface for different user roles.
Structure a centralized patient monitoring system.
Design relationships between healthcare-related database entities.
Plan personalized discharge workflows.
Design medication and recovery tracking systems.
Develop follow-up scheduling and reminder workflows.
Design risk-alert mechanisms based on patient-reported information.
Think about usability, security, scalability, and continuous patient monitoring.
The project also helped us understand the importance of keeping patient information centralized so healthcare professionals can access relevant post-operative information more efficiently.
🚧 Challenges We Faced
One of the major challenges was designing a system that could handle different aspects of post-operative care without making the interface complicated for patients.
We also had to consider how information from different modules—such as medications, symptoms, recovery records, appointments, and alerts—could be connected and presented clearly.
Another challenge was designing an effective risk-alert workflow. Patient-reported symptoms need to be organized in a way that can help healthcare professionals identify patients who may require attention.
We also focused on making the system usable for both patients and healthcare professionals while keeping their workflows different and easy to understand.
🚀 Our Vision
CareTrack is designed as a foundation for a more connected post-operative care experience.
By combining discharge planning, medication monitoring, recovery tracking, follow-up management, patient self-reporting, and risk alerts in one platform, we aim to make post-operative care more continuous and organized.
In the future, the platform could be extended with additional healthcare integrations, advanced analytics, notification services, and more intelligent risk-assessment capabilities.
