Inspiration

Stroke patients often face long waits for follow-ups, fragmented care and uncertainty about where to turn next. We wanted to build something that restores hope, shortens the recovery gap and gives patients and families clarity and confidence in navigating rehab.

What it does

Carematch helps stroke patients and their families take control of the recovery journey by bridging the gap between hospital discharge and community care. The platform makes it simple to:

  • Upload and manage referrals and medical documents
  • Match with the most suitable providers based on expertise, availability, cost, and location
  • Book appointments seamlessly and keep track of progress In short, Carematch turns a fragmented and stressful process into a clear, guided path to recovery.

How we built it

We spoke to stroke patients and healthcare professionals to understand their experiences and the gaps in the post-recovery journey. With these insights, we developed a quick prototype using Google Stitch and Figma. As more conversations unfolded, we refined the solution to better align with patient and provider needs.

Challenges we ran into

  • Approaching conversations with post-stroke patients required care and sensitivity, as we wanted to learn from their experiences without overwhelming them
  • Discovering that a similar app, Stroke Buddy, already exists initially raised concerns about overlap. Instead of seeing it as a setback, we reframed it as validation of the need and used it to sharpen Carematch’s differentiation. We focused not on self-management, but on solving the harder problems of access, coordination and transparency in recovery.

Accomplishments that we're proud of

  • Defined a clear value proposition that goes beyond existing self-management apps by addressing access and coordination
  • Designed a patient-first experience that simplifies what is often an overwhelming recovery journey
  • Stayed agile as a team, through pivoting quickly after discovering similar apps and sharpening our differentiation in response

What we learned

  • The presence of similar tools is not a weakness but rather a validation of the need in this space.
  • What truly differentiates Carematch is addressing the systemic gaps in access and coordination, not just self-management
  • Clearly defining our positioning against existing solutions made our pitch sharper, more credible, and easier to understand
  • Building with input from both patients and healthcare professionals is essential to create a solution that is both practical and trusted

What's next for Carematch

  • Expand beyond stroke to support other rehabilitation and chronic care journeys
  • Scale from GPs into Polyclinics to strengthen continuity of care in the community
  • Integrate with insurance platforms to simplify financial coverage and claims
  • Develop AI-driven matching for specialists and services to personalize recovery
  • Enable cross-border care matching to support international patients seeking treatment in Singapore
