Inspiration

Healthcare often breaks down not at the doctor's visit, but in the gap between visits — monitoring, waiting for reports, raising concerns, completing follow-ups. CareLoop connects patients and hospital care teams through one structured, trackable workflow, so information doesn't just get received — it reaches the right team, triggers action, and stays tracked until the loop closes.

What it does

CareLoop is a hospital-connected platform managing care between visits. Hospitals enroll patients and assign them a Care Plan defining what to monitor and how often. Patients can submit health measurements, view diagnostic reports, raise concerns (text/voice), and get follow-up from their care team. Care teams get a dashboard to review, assign, communicate, and track each case to closure.

Workflow: Care Plan → Submit/Receive → Route → Review → Action → Follow-up → Close

How we built it

Two connected interfaces — a mobile-friendly Patient Portal and a workflow-oriented Care Team Dashboard — built around the hierarchy Hospital → Department → Care Team → Patient → Care Case. Every important interaction generates a Care Case with priority, assigned team, status, and timeline. AI acts as an assistive layer (e.g., turning voice concerns into structured summaries) — never for autonomous diagnosis or treatment decisions. The prototype uses simulated data.

Challenges we ran into

• Avoiding an overloaded patient portal → solved with a unified Care Case system for updates, reports, and concerns alike
• Different patients need different monitoring → solved with a configurable Care Plan
• Privacy/access control → solved by separating hospitals, departments, teams, and patients rather than one shared account
• Defining AI's role → limited it to organization and summarization, not clinical decisions

Accomplishments we're proud of

Turning several disconnected healthcare problems into one unified system built around the full care journey (update → case → review → action → follow-up → closure), especially the Care Plan Builder and the Care Case concept, demonstrated across both patient and care-team sides.

What we learned

Good healthcare tech isn't about more features — it's about a workflow that's simple for patients and useful for clinicians. Role-based access, personalized care plans, and structured information matter more than free-form communication. AI works best supporting professionals, not replacing them. Healthcare is a continuous process, not a single visit.

What's next

Move from prototype to deployable platform: EHR/hospital system integration, interoperability with India's digital health ecosystem, more configurable care plans, connected device integration, stronger analytics, multilingual/voice-first interfaces, offline support, and deeper AI-assisted workflows.
