The frustration of navigating fragmented healthcare systems — patients unsure where to go, GPs overwhelmed with referrals, and specialists receiving incomplete intake information.

What it does

CareFlow OS is a full-stack care coordination platform. Patients complete an AI-powered intake form, receive an instant clinical triage, and choose a specialist directly. Specialists log in with their credentials to manage assigned referrals and update patient status through a dedicated dashboard. Automated emails keep patients informed at every step.

How we built it

Next.js 16, Gemini AI for triage, Supabase for persistent real-time data, Resend for transactional emails, and Vercel for deployment.

Challenges we ran into

Getting the AI triage to return reliable structured JSON, migrating from in-memory stores to a live Supabase database mid-build, and designing a specialist assignment flow that felt natural for both patients and clinicians — all under hackathon time pressure.
