Inspiration
After talking to research reports and hearing stories of patients newly diagnosed with chronic conditions, I realized how overwhelming it is to manage healthcare, insurance benefits, lifestyle changes, and emotional wellbeing all at once.
Many patients leave hospitals confused, unsure what their Sun Life insurance covers, and isolated without community support. I wanted to build CareConnect AI to bridge this gap — a single, compassionate companion to guide patients step by step.
This project taught me how AI can be applied to real-world healthcare challenges, and how to think from the patient’s perspective. It also challenged me to design a solution that is both empathetic and practical.
About the Project
CareConnect AI is an AI-powered web app that helps newly diagnosed chronic patients at Waterloo Hospital:
Understand and navigate their Sun Life insurance benefits
Follow a personalized lifestyle and care plan
Access emotional support and community resources
I built it using Streamlit and Python, creating a live prototype with interactive tabs for benefits, care plans, and community support. Visual mockups for slides were designed using Figma, with colors and fonts chosen to evoke calm, trust, and empathy.
Challenges faced:
Making a solution that feels human and compassionate, not robotic
Integrating multiple features (benefits clarity, lifestyle guidance, emotional support) into a simple and easy-to-follow interface
Designing visuals quickly with limited time, while keeping professional quality
