Inspiration
Our project was inspired by the real struggles that university students newly diagnosed with chronic conditions face when trying to manage their health. Many feel overwhelmed after receiving a diagnosis uncertain about their insurance coverage, unsure how to coordinate care, and struggling to balance academic life with new lifestyle changes.
We wanted to create something that would make this transition less confusing and more empowering. That’s how Care Bridge was born a single, integrated app that connects users to their insurance benefits, lifestyle tools, and long-term care plans, all in one place.
For this project, our team was given five personas, each representing a different chronic condition. We chose to focus on the persona diagnosed with endometriosis.
This focus allowed us to design features and resources that address the unique challenges faced by users managing this condition such as understanding fertility-related benefits, coping with chronic pain and fatigue, and balancing academic and personal responsibilities.
While our prototype centers on endometriosis, the framework of Care Bridge is intentionally scalable and inclusive capable of adapting to support other chronic conditions in future iterations.
What it does
Care Bridge is an integrated digital health platform that helps newly diagnosed Sun Life clients understand their benefits, manage daily lifestyle changes, and navigate long-term care all in one place.
- Mathematically speaking, our goal was to bring three separate variables (Benefits)+(Lifestyle)+(Long-term Care) into a single, cohesive equation of clarity, accessibility, and support.
By bridging these elements, we reimagined Sun Life’s role from a reactive payer to a proactive health partner guiding clients toward confidence and control.
How we built it
We started by analyzing Sun Life’s current digital ecosystem and identifying the missing connections between benefits, healthcare, and daily management. Using user journey mapping, wireframes, and prototyping tools like Figjam and Figma make, we created an app that allows users to:
Understand their benefits – with a clear breakdown of what’s covered.
Manage lifestyle changes – through condition-specific support and tools.
Navigate long-term care – by tracking appointments and next steps.
To make the experience seamless, we focused on a simple and intuitive navigation system that keeps the interface approachable for all users.
Challenges we ran into
Our main challenge was merging three major problems benefits understanding, lifestyle management, and long-term care navigation into one user-friendly app. We also had to ensure the app was inclusive, adaptable for all chronic conditions, and not limited to a single diagnosis. Balancing simplicity with depth was tricky, but it pushed us to refine our design so that users always feel guided, not overwhelmed.
Accomplishments that we're proud of
We’re proud that our team successfully merged three complex challenges into one cohesive, user-friendly experience. Designing a solution that simplifies such a layered problem while remaining inclusive for all chronic conditions was a major achievement.
We’re also proud of how intuitive and navigable our app turned out to be. Through user journey mapping and interface testing, we created a flow that feels clear, supportive, and approachable — especially for users feeling overwhelmed by their diagnosis.
Finally, we’re proud that Care Bridge goes beyond being just an app — it redefines Sun Life’s role from a financial provider to a proactive health partner, empowering users to take control of their care with clarity and confidence.
What we learned
Throughout this project, we learned the importance of mapping the real user journey before jumping into solutions. Understanding how people move from diagnosis to management helped us see that the problem wasn’t a lack of resources it was that everything felt fragmented and disconnected.
We also discovered how crucial it is to design for inclusivity. Every chronic condition brings unique needs, but accessibility and clarity benefit all users.
What's next for Care Bridge
Looking ahead, our next step is to expand Care Bridge beyond a single condition and make it adaptable for all chronic conditions supported by Sun Life’s client base.
We plan to:
Integrate AI personalization: allowing the app to recommend resources, track progress, and tailor care pathways based on each user’s diagnosis and habits.
Enhance accessibility features: ensuring inclusivity for users with varying needs through voice assistance, multilingual support, and adaptive interfaces.
Build stronger healthcare integrations connecting directly with hospital systems, pharmacies, and care providers for real-time updates and appointment management.
Introduce community and peer support features creating a safe space where users can connect with others living with similar conditions.
Launch pilot testing with real Sun Life users and Waterloo Hospital patients to gather feedback and refine the app’s usability and impact.
Ultimately, our goal is to transform Care Bridge into a comprehensive, AI-powered health companion that grows with the user helping them navigate every step of their care journey with clarity, confidence, and compassion.
