Andrey Ostrovsky, MD and Jeffrey Levy each had bad experiences with family members receiving an unsatisfactory level of support as they entered a period of decreasing independence. They saw an opportunity to enable low-cost care providers such as home care attendants who assisted with activities of daily living to detect early health decline to keep their clients healthy and in their homes. Over the years the technology has developed into a simple survey tool, supported by best practices of care coordination and on-going risk-stratification based on the client's recent activities and current situation. We are most proud of our success stories which define early health decline which might have otherwise led to an ED visit or hospitalization. The granular data that we have generated has also enabled us to work with our customers to reach their goals using industry-leading quality improvement processes.
