Inspiration
The Singapore government has been successful in the consolidation of inter-cluster electronic medical records including patients' list of medications with a focus on how data can be transferred securely and efficiently. With our current ageing population in Singapore projected to be a super-aged society in 2026, there is also increasing digital literacy of citizens including the elderly. Polypharmacy and drug interactions are prevalent issues that still persist in healthcare as more patients are diagnosed with chronic illnesses and co-morbidities, and have more medications prescribed. There are also multiple processes involved to retrieve their medications.
Current portals do not provide a one-stop solution for management of medications where patients can only access their medications in segregated segments that require separate retrieval from HealthHub and from different healthcare clusters. The DIY medication reminders on current apps do not automatically input the very long list of medications but more so by the patient so there is a risk of error during inputing.
What it does
We have a 3-in-1 integrated app to consolidate all things related to medication management, made accessible to all patients to increase convenience and ease of life. We want to make the value chain of the medication supply more integrated and smoother, and to manage and track the active list of medications for each patient effectively.
Our proposed workflow of the app is to:
- Set automated medication reminders to reduce errors - The list of medication prescriptions will be compiled from HealthHub and ported over to our app for automated reminders.
- Medication refill and delivered to your door step - This completes the entire set of processes associated with managing medications for chronic conditions such has Type 2 diabetes, stroke prevention, lowering cholesterol.
- Book a pharmacist tele-consultation appointment through the app with Singpass - This allow for medication counselling with a pharmacist by helping the patient to sort out his/her medications through an authenticated and safe environment. Function 1 and 2 combine all medications in one interface for a better user experience to reduce duplication.
The potential impacts are that this app can provide more clarity for doctors, pharmacists, patients and their caregivers. This in turn can increase the sense of fulfilment in pharmacists through direct and purposeful interactions with patients.
What sets us apart is that while most medication apps aim at reminding patients to take their medications with some including health tracking functions and information on potential drug interactions, our solution optimises the current healthcare information landscape for patients to know, take and get the right medications, and also encourages patients to tap on services provided by pharmacists.
How we built it
We drafted the ideas on Canva and designed the UIUX of the prototype app using Figma.
Challenges we ran into
We found it difficult to pinpoint the specific healthcare problem we would like to solve. We explored the Cogniss app and realised that it is unsuitable in terms of the functions we were planning to use.
Accomplishments that we're proud of
We managed to think out of the box and come up with this idea for a one-stop integrated solution that is currently not seen in the market.
What we learned
We learnt to solve smaller but specific problems instead of grand but vague problems and we also learn more on UI/UX designing as a useful skill.
What's next for
We are looking forward for the solution to be pushed out by getting approval for integration with HealthHub, getting pharmacist partners, getting Singpass authorisation and ultimately building the app.
