Inspiration
We built AfterCare after seeing how difficult it was for our own grandparents to understand hospital discharge paperwork. Important instructions about medications, follow up appointments, and warning signs were buried in pages of dense medical language. While anyone leaving the hospital can benefit from clearer discharge instructions, we designed AfterCare with older adults and their caregivers as our primary focus because they are often the most affected by these challenges.
What it does
AfterCare turns hospital discharge paperwork into a personalized recovery plan. Patients can upload a PDF or photo of their discharge documents, and AfterCare organizes the important information into medications, appointments, warning signs, and a recovery timeline. It also includes grounded question answering, source citations, and text to speech support. Although the platform is designed with accessibility features that make it especially useful for older adults, it is built to help patients of all ages better understand their recovery instructions.
Challenges we ran into
Medical documents vary significantly in formatting, making information extraction difficult. We also wanted to avoid hallucinations, so we spent a lot of time building a verification pipeline that checks every answer against the original document. Another challenge was designing an interface that is simple enough for older adults while still being useful and efficient for patients and caregivers of all ages.
Accomplishments that we're proud of
We created a complete end to end application that transforms complicated discharge paperwork into a simple recovery guide in seconds. We're especially proud of our grounded AI pipeline, fail safe design that avoids inventing medical information, and accessibility features like text to speech, large text, and high contrast. These features make AfterCare especially effective for older adults while keeping the experience useful for every patient recovering at home.
