The Night Everything Changed

In October 2025, a growing, unexplainable pain in my lung sent me to the ER. I had no idea what was coming next.

I was diagnosed with a pneumothorax (a collapsed lung). After having a chest tube inserted, I spent 10 days across two hospitals, eventually transferring to a larger facility for surgery.

The total bill: $80,000.

I panicked. Thankfully, I qualified for emergency Medicaid, and I assumed the worst was over and everything was covered. Then, the real nightmare started.


The Process Was Worse Than the Illness

A month after I returned home, the mail started piling up. Bills from the ER, the anesthesiologist, the radiologist, each from a completely separate billing department. Each bill required a separate phone call, sitting on hold for at least 30 minutes, strictly between the hours of 9 to 5.

Nobody warned me that I had to call every single department individually to verbally request they run my insurance. When I finally navigated the hold times, two weeks would go by with no action. I had to call again. I was discharged in November 2025. The billing process dragged on until April 2026.

The hardest part wasn't the surgery. It was the financial bureaucracy nobody prepared me for.

I am someone who knows how to search the internet, use AI tools, and confidently navigate complex systems in English. And even I was completely overwhelmed.

This highlighted a profound social and financial inequity:

  • Marginalized Communities: Immigrant parents who don't speak English have no way to decipher these documents or fight these charges.
  • Financial Accessibility: Full-time, hourly workers cannot afford to spend 30+ minutes on hold during regular business hours to dispute a claim.
  • The Wealth Gap: Professional "patient advocates" exist to handle this, but at $200/hour, they are a luxury reserved for the wealthy.

We decided to build that $200/hour financial advocate. For free. For everyone.


What We Built: Advocate AI

Advocate AI is an autonomous, AI-powered financial advocate that democratizes healthcare billing. It reads your medical bills, extracts the financial data, and makes the grueling insurance calls for you.

How It Works (Powered by Gemini & ElevenLabs)

  1. 📄 Upload & Analyze: Snap a photo of any medical statement.
  2. 🔍 Data Extraction (Google Gemini API): We leverage Gemini's multimodal capabilities to act as a supercomputer, instantly extracting the provider name, claim number, CPT codes, and the balance due from complex medical documents.
  3. 🎙️ Voice Synthesis (ElevenLabs API): The user records a short sample, and ElevenLabs creates a highly realistic, emotionally expressive voice clone to act as their proxy.
  4. 📞 Autonomous Call: Our AI agent dials the billing department using the cloned voice and autonomously navigates complex call flows.
  5. 📊 Live Dashboard: Users watch the real-time transcription unfold as the call happens.
  6. 📝 Financial Summary: Get a plain-English breakdown of the outcome, claim status, and actionable financial next steps.
  7. 💬 Claim Assistant Chat: An interactive chatbot that provides personalized financial literacy advice regarding the user's specific medical debt.

Technical Moonshots & Challenges

  • Real-time Calling Agent: Connecting a live audio transcription to make a call demanded careful architectural planning and AI integrations.
  • Seamless Audio Generation: Integrating ElevenLabs required ultra-low latency to ensure the autonomous voice interactions felt perfectly human.

The Impact

We learned that the American healthcare billing system is fundamentally broken. Advocate AI isn't just a technical hack; it is a scalable solution for social good. By combining Gemini's reasoning with ElevenLabs' voice capabilities, we are giving underserved patients the tools they need to protect their financial health.


What's Next

  • Multilingual Support: Adding Spanish, Korean, and Chinese to bridge the language gap for immigrant families.
  • Auto-Generated Appeals: Automatically drafting PDF legal and financial appeals based directly on call transcripts.
  • Expanded Fintech Features: Integrating direct payment solutions and smart retry schedulers to handle dental, vision, and pharmacy claims.
